Job Summary

Key Responsibilities

Perform telephonic social work consultations with members to promote community resources
Document clinical consultations and resources provided in the internal database
Serve as a clinical resource for other clinicians and participate in staff development opportunities

Required Qualifications

Master's Degree in Social Work (LSW)
Professional License in good standing for respective discipline
2+ years of professional experience in a social work position with adults in the Medicaid/Medicare population
Experience with arranging community resources
Knowledge of the healthcare delivery system
