Licensed Social Worker, MO/IL

Job Summary

A company is looking for a Social Worker to address the psychosocial and socioeconomic needs of members in a remote setting.

Key Responsibilities

• Coordinate psychosocial services and assist members with medical resource utilization
• Conduct needs assessments to identify barriers and recommend appropriate resources
• Participate in case conferences to develop member-specific care plans and maintain documentation

Required Qualifications

• LCSW and MSW required
• Licensed in Missouri and/or Illinois, eligible for licensure in other states
• Minimum of 2 years of clinical experience
• Experience in a managed care environment preferred
• Computer proficiency
