Licensed Value Based Care Specialist

Job Summary

A company is looking for a Value Based Care Specialist.

Key Responsibilities:

• Interface with patients through various communication methods to support chronic disease management
• Provide education and coordination for patients regarding their chronic illness management
• Collaborate with healthcare teams to ensure high-quality patient assessments and care plans

Required Qualifications:

• An LVN/LP or MA degree & certification with at least 2 years of relevant work experience
• Familiarity with Chronic Care Management, Remote Patient Monitoring, or similar programs
• 1-2 years of experience using Electronic Health Records (EHR) software
• Ability to manage patient healthcare needs via various communication methods
• Knowledge of healthcare terms and basic care principles
