Licensed Virtual Care Nurse Practitioner

Job Summary

Providing holistic, evidenced-based care, the full-time Licensed Virtual Care Nurse Practitioner will deliver patient services remotely, including assessment, diagnosis, treatment, and health counseling for patients aged 18 months and above, with a schedule that includes every other weekend.

Key responsibilities:

• Deliver accurate assessment, diagnosis, treatment, and management of health problems while providing health counseling and disposition planning
• Evaluate primary care and transitional care patients, offering healthcare education and counseling
• Document patient care within an EHR and coordinate care with internal and external colleagues to ensure high standards of care

Required qualifications:

• Current unrestricted license in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV
• Minimum of two years of medically relevant experience or equivalent
• Completion of a Master's Degree level Family Nurse Practitioner program with current National Board Certification
• Ability to work autonomously and collaborate effectively with healthcare teams
• Proficiency in information management and technology
