Job Summary

A company is looking for a Licensed Vocational Nurse or a Licensed Practical Nurse to join their Primary Care team.

Key Responsibilities:

• Manage patients' healthcare needs through various communication methods
• Assist clinicians with administrative tasks and patient care coordination
• Identify barriers to optimal member health and implement strategies to reduce risks

Required Qualifications:

• Graduate of an accredited School of Practical/Vocational Nursing (LPN, LVN)
• Active, unrestricted LVN/LPN license in specified states
• Experience in virtual-based practice
• Spanish language fluency
• 3+ years of in-person outpatient or ambulatory LVN/LPN experience
