Job Summary

A company is looking for a SS Afterhours Care Coordinator.

Key Responsibilities

• Documents incoming communication and coordinates services on approved reporting forms
• Makes patient care decisions and provides guidance to home care clinical staff regarding patient care issues
• Coordinates and assigns visits to home care based on patient needs and monitors high-risk patient reports

Required Qualifications

• Current and unencumbered license to practice as a Registered Nurse
• Three years of experience as a Registered Nurse, including one year in home health/hospice
• Current CPR certification
• Knowledge of Federal/State rules and regulations, ACHC standards, and other regulatory requirements
• Strong fiscal planning and management skills
