Job Summary
To coordinate and manage behavioral health services for members enrolled in Medicare and Commercial insurance plans, the full-time Louisiana Licensed Behavioral Health Case Manager will conduct assessments, develop care plans, and provide crisis intervention, working both remotely and in-person as needed.
Key responsibilities
- Conduct comprehensive behavioral health assessments and develop individualized care plans in collaboration with members and providers
- Coordinate behavioral health services across various care settings and facilitate transitions after hospitalizations or emergencies
- Participate in crisis intervention by managing calls and providing immediate support and risk assessment as part of a 24/7 crisis line
Required qualifications
- Bachelor's degree in nursing, healthcare, or a related field, or a master's degree in social work, counseling, or behavioral health
- RN license in Louisiana, or LPN license with 2 years of related experience, can substitute for the bachelor's degree
- Minimum of three years of experience in case management, specifically in behavioral health or crisis intervention
- Active LPC, LCSW, LMFT, RN, or LPN license required
- Strong understanding of behavioral health diagnoses and treatment modalities, along with proficiency in electronic health records
