Louisiana Licensed Care Coordination Nurse

Job Summary

Providing telephonic clinical assessments and health education, the full-time Louisiana Licensed Care Coordination Nurse will assist in directing patients to appropriate care levels while working remotely from Louisiana or Mississippi.

Key responsibilities

• Conduct telephonic clinical assessments and nursing triage for a diverse patient population
• Assist in directing patients to the most appropriate level of care and ensure coordination with team members
• Perform outbound follow-up calls and participate in the Care Management process

Required qualifications

• High School Diploma required
• LPN License required, with a multistate license under the NLC preferred
• Experience in Home Health, Med Surg, or Emergency Department preferred
• Advanced computer skills necessary
• Ability to work in the Eastern time zone, with shifts starting at 8:30am EST
