Job Summary

A company is looking for a Care Management Associate to manage enrollment and support comprehensive coordination of medical services for population health members.

Key Responsibilities

• Manage enrollment for child and family welfare population health members and carry a low tier caseload
• Coordinate and implement Wellness Plans, including community resource engagement and care gap reviews
• Conduct initial reviews and triage Care Team tasks while ensuring compliance with regulatory standards

Required Qualifications

• Must reside in Louisiana
• 2-4 years of experience in healthcare or working with foster, child and family welfare populations
• High School Diploma or G.E.D. is required
• Proficiency with personal computers and MS Office Suite applications
• Flexibility to work occasional nights and weekends outside of standard business hours
