Louisiana Licensed Care Manager RN

Job Summary

Supporting a diverse member population remotely, the full-time Louisiana Licensed Care Manager RN will make outbound and receive inbound calls to assess health statuses, identify treatment gaps, and provide patient education while ensuring a continuum of quality care.

Key responsibilities:

• Assess members' health status through phone interactions and identify barriers in treatment plans
• Provide patient education and make referrals to external resources to enhance self-management
• Maintain close communication with members to improve health outcomes and reduce utilization

Required qualifications:

• Active Multistate Compact Nursing License with residency in Louisiana
• 3+ years of experience in a hospital, acute care, or direct care setting
• Proficiency in navigating a Windows-based environment and typing skills
