A company is looking for a Remote Clinical Pharmacist licensed in Louisiana.

Key Responsibilities

• Conduct telephonic consultations and schedule member appointments
• Review prescription claims data to identify gaps in care and adherence issues
• Assess medication regimens for appropriateness, effectiveness, and safety

Required Qualifications

• Bachelor's Degree in Pharmacy or PharmD
• Current, unrestricted Louisiana Pharmacist license
• Strong knowledge of MTM guidelines and disease state management
• Remote work experience and excellent organizational skills
• Flexibility to work rotating shifts
