Job Summary
To enhance patient care quality, the part-time Louisiana Licensed Clinical Pharmacist will provide integrated medication therapy management for diabetes and obesity patients, collaborating remotely with a hybrid team to ensure effective drug therapy and adherence.
Key responsibilities
- Provide medication therapy management, including medication reconciliation and adherence counseling for diabetes and obesity patients
- Collaborate with the interdisciplinary care team to develop individualized care plans based on evidence-based guidelines
- Monitor medication regimens to ensure compliance with clinical criteria and identify opportunities for drug therapy optimization
Required qualifications
- Doctor of Pharmacy (PharmD) and a valid Louisiana pharmacist license
- Minimum of 3 years of experience as a clinical pharmacist; completion of PGY1/PGY2 residency is preferred
- Strong clinical knowledge in diabetes care and disease state management
- Ability to analyze information and exercise good judgment in clinical settings
- Excellent communication skills and a strong focus on patient safety and medication literacy
