Job Summary
To enhance patient care within diabetes and obesity management, the part-time Louisiana Licensed Clinical Pharmacist will provide medication therapy management, collaborate with healthcare teams, and conduct clinical assessments while working remotely.
Key responsibilities
- Provide medication therapy management, including medication reconciliation and adherence counseling for patients with diabetes and obesity
- Collaborate with interdisciplinary care teams to develop individualized care plans and ensure compliance with clinical guidelines
- Monitor medication regimens and participate in quality improvement initiatives to enhance clinical outcomes and medication management
Required qualifications
- Doctor of Pharmacy (PharmD) and a valid pharmacist license in Louisiana
- Minimum of 3 years of experience as a clinical pharmacist; PGY1/PGY2 residency preferred
- Strong clinical knowledge in diabetes care and disease state management
- Proficient in Microsoft Office tools and data management
- Ability to maintain confidentiality and handle sensitive information with discretion
