Job Summary

A company is looking for an LPN Care Navigator to manage patient care and provide health coaching remotely.

Key Responsibilities

• Manage a panel of 400-450 patients with monthly outreach for continuity of care
• Conduct 50-60 daily outbound calls to provide care plan support and health coaching
• Document patient interactions in real time while maintaining HIPAA compliance

Required Qualifications

• Active Licensed Practical Nurse or Licensed Vocational Nurse license with a multi-state compact license
• Minimum of 2 years' clinical experience
• Experience working with EMR systems
• Familiarity with chronic condition management and individualized care planning
• Intermediate to advanced computer skills for navigating multiple systems
