Louisiana Licensed Nurse Case Manager

Job Summary

A company is looking for a Case Manager, Registered Nurse (Remote).

Key Responsibilities

• Assess, plan, implement, and coordinate case management activities for members to evaluate their medical needs
• Develop proactive care plans to enhance member wellness and address identified health issues
• Utilize clinical tools to evaluate member needs and conduct assessments considering various health conditions

Required Qualifications

• Active and unrestricted RN licensure in Louisiana or compact licensure in state of residence
• 3+ years of clinical experience
• 2+ years of case management, discharge planning, or home healthcare coordination experience
• Associate's Degree in Nursing (required); Bachelor's Degree (preferred)
