Job Summary
Providing compassionate telephonic care, the part-time Louisiana Licensed Nurse Practitioner will deliver after-hours support to aging residents across multiple states, managing high call volumes and conducting telehealth visits to address acute and chronic health needs.
Key responsibilities
- Serve as the first line of support for residents and facility staff after-hours, offering medical care and direction over the phone
- Utilize Curana's telephonic platform to coordinate care between facilities, hospitals, and clinics while documenting encounters in the EMR
- Deliver high-quality, cost-effective care in collaboration with physicians and specialty providers, applying clinical protocols and practice guidelines
Required qualifications
- Master's Degree as a Nurse Practitioner
- Active, unrestricted licensure in Louisiana; additional licenses in other states preferred
- Nurse Practitioner national certification as ANP, FNP, or GNP
- 3+ years of experience as a Nurse Practitioner with a background in acute and chronic disease management
- Proficient computer skills, including documentation in written and electronic medical records
