Job Summary
To support health outcomes for at-risk members, the full-time Louisiana Licensed Population Health RN will organize, coordinate, and provide care management services remotely, focusing on improving health and financial outcomes through comprehensive assessments and interventions.
Key responsibilities
- Assess member status and develop comprehensive case management plans based on individual needs
- Coordinate care by collaborating with members, providers, and community resources to achieve healthcare goals
- Evaluate and modify case management plans to ensure effectiveness in reaching desired health outcomes
Required qualifications
- Active and unencumbered RN license to practice in Louisiana
- Diploma, Associate's, or Bachelor's degree in nursing
- 3 years of recent direct patient care/clinical experience
- 2 years of experience in managed care is preferred
- Certified Case Management Certification is preferred, with a requirement to obtain it within 3 years from date of hire
