Job Summary

The full-time, remote Louisiana Licensed Population Health RN will organize, coordinate, and provide care coordination and case management services to high-risk members, focusing on improving health outcomes through collaborative assessment and intervention planning.

Key responsibilities

• Assess and develop comprehensive case management plans based on individual member needs
• Implement and evaluate case management activities to achieve established health goals
• Collaborate with members, providers, and community resources to facilitate access to necessary healthcare services

Required qualifications

• Active and unencumbered RN license to practice in Louisiana
• Diploma, Associate's, or Bachelor's degree in nursing
• 3 years of recent direct patient care/clinical experience
• Experience in managed care is preferred
• Certified Case Management Certification is preferred, required within 3 years of hire
