Job Summary

To support patients and families through complex health journeys, the full-time Louisiana Licensed RN Case Manager will work remotely, utilizing clinical expertise to create personalized care plans, coordinate care across providers, and empower members with education and support.

Key responsibilities

• Building meaningful relationships with patients and families to understand their goals and barriers
• Creating personalized care plans that address medical, behavioral, and social needs
• Coordinating care across various providers and settings to ensure quality and continuity

Required qualifications

• Minimum of 5 years as a Registered Nurse
• Active Compact RN License
• Strong background in care management and chronic condition management
• Skilled in motivational interviewing and patient education
• Must be local to St. Tammany Parish, LA
