Job Summary

A company is looking for an RN Transition Specialist to support the coordination of member transitions in the community.

Key Responsibilities

• Support care coordination teams and providers in developing effective transition plans for members
• Assist with member transitions based on enrollment or care service needs
• Track transition metrics and ensure proper documentation of care transitions

Required Qualifications

• Bachelor's degree required
• 2 - 4 years of related experience preferred
• Equivalent experience may be considered
• Residency in Louisiana is required
• Experience with care coordination and transition planning is beneficial
