Job Summary

Serving as the primary point of contact for patients, the full-time LPN Care Advocate will provide non-clinical navigation support remotely, helping patients understand their care plans, schedule appointments, and connect with community resources.

Key responsibilities
  • Assist patients in understanding and acting on their care plans, including scheduling appointments and navigating Medicare benefits
  • Resolve navigation issues related to referrals, appointment logistics, and provider communications
  • Document all patient interactions and support activities accurately and in a timely manner
Required qualifications
  • Active, unrestricted LPN/LVN license in good standing, Community Health Worker certification, or equivalent experience
  • 2+ years of patient- or community-facing support experience
  • Working knowledge of the U.S. healthcare system and Medicare
  • Ability to manage a caseload and maintain strong documentation independently
  • Reliable computer, internet access, and a quiet space for patient calls
