Job Summary
Serving as the primary point of contact for patients, the full-time LPN Care Advocate will provide non-clinical navigation support remotely, helping patients understand their care plans, schedule appointments, and connect with community resources.
Key responsibilities
- Assist patients in understanding and acting on their care plans, including scheduling appointments and navigating Medicare benefits
- Resolve navigation issues related to referrals, appointment logistics, and provider communications
- Document all patient interactions and support activities accurately and in a timely manner
Required qualifications
- Active, unrestricted LPN/LVN license in good standing, Community Health Worker certification, or equivalent experience
- 2+ years of patient- or community-facing support experience
- Working knowledge of the U.S. healthcare system and Medicare
- Ability to manage a caseload and maintain strong documentation independently
- Reliable computer, internet access, and a quiet space for patient calls
