LPN Care Coordination Nurse

Job Summary

Providing telephonic clinical assessments and patient education, the full-time LPN Care Coordination Nurse will assist in directing patients to appropriate care levels while working remotely across the United States.

Key responsibilities

• Conduct telephonic clinical assessments and nursing triage for a diverse patient population
• Assist in directing patients to the most appropriate level of care and ensure adequate coordination with team members
• Perform outbound follow-up calls and participate in the Care Management process

Required qualifications

• High School Diploma required
• LPN License and Multistate License under the NLC required
• Experience in Home Health, Med Surg, or Emergency Department preferred
• Completion of nursing training as an LPN
• Ability to work Eastern Time Zone hours (Monday - Friday, 8 hour shifts starting at 8:30am EST)
