LPN Care Coordinator

Job Summary

A company is looking for an LPN Care Coordinator (1099 Contract | Fully Remote).

Key Responsibilities

• Review patient EMR records to determine eligibility for care management services
• Make proactive patient outreach calls to provide support, education, and resource connections
• Develop and maintain individualized care plans, ensuring timely and appropriate patient care

Required Qualifications

• LPN Compact License (or ability to obtain one if needed)
• 5+ years of patient-centered care experience with strong clinical knowledge
• Proficiency in EMR systems and comfort in a remote, digital environment
• Fluency in Spanish is preferred
• Self-motivated and detail-oriented with a focus on performance targets
