LPN Care Coordinator

Job Summary

Joining a dedicated team, the fully remote LPN Care Coordinator will manage patient interactions, educate clients about palliative care, and handle administrative tasks while ensuring high-quality care for seniors with serious illnesses.

Key responsibilities

• Educate patients about the value of palliative care and schedule appointments
• Document patient encounters and triage patient needs effectively
• Review and close completed cases daily while balancing clinical triage and patient calls

Required qualifications

• Minimum 3+ years as an LPN
• Minimum 3+ years of experience reading and updating clinical notes
• At least 1+ years of relevant experience in a customer service call center
• Strong communication skills and comfort with technology
• Secure Wi-Fi connectivity; Spanish-speaking is a plus but not required
