LPN Care Coordinator

Job Summary

Working remotely on a full-time basis, the LPN Inbound Care Coordinator will ensure high-quality care services by managing patient care plans, coordinating with clinicians and patients, and maintaining effective communication to enhance healthcare outcomes.

Key responsibilities:

• Complete individualized patient care plans and perform care management using the electronic medical record system
• Maintain frequent contact with patients to provide support and manage compliance with care management programs
• Document all client communications accurately and keep the Team Supervisor informed of pertinent issues

Required qualifications:

• Must be a Licensed Practical Nurse (LPN)
• Ability to work a weekend schedule and specific weekday hours
• Proficiency in using computers and Microsoft Office programs
• Compassionate and empathetic with exceptional customer service skills
• Ability to multitask and thrive in a fast-paced environment
