LPN Care Coordinator - Remote

Job Summary

To provide essential care coordination services, the full-time LPN Care Coordinator - Remote will manage patient care plans, communicate with healthcare teams, and monitor patient progress while working from home.

Key responsibilities:

• Develop and maintain individualized care plans for patients, ensuring timely management of care activities
• Conduct monthly care calls and educate patients on chronic conditions and treatment plans
• Monitor patient progress and document all relevant information in electronic medical records

Required qualifications:

• Bilingual in English and Spanish
• Minimum of two years of experience providing patient-centered care
• At least one year of experience in skilled nursing
• Proficient in working with electronic medical records
• Ability to perform nursing assessments and develop care plans
