LPN Care Coordinator (Spanish)

Job Summary

To support a growing healthcare service, the full-time LPN Care Coordinator (Spanish) will manage inbound clinical phone calls from patients, assist in triaging clinical issues, and engage with patients through various communication channels in a fully remote setting.

Key Responsibilities:

• Manage inbound calls and engage with patients to address inquiries
• Assist in triaging clinical issues and connect patients with the Triage Nurse team
• Support the Provider Relations Specialists team with patient and provider communication

Required Qualifications:

• Current active, compact LPN/LVN license
• Bilingual and fluent in Spanish
• LPN/LVN degree from an accredited nursing program
• Excellence in following established clinical protocols and procedures
• Flexibility and adeptness in switching tasks effectively
