LPN Care Coordinator - Spanish Required

Job Summary

A company is looking for an LPN Care Coordinator with Skilled Nursing Experience (Spanish Speaking Required).

Key Responsibilities

• Develop and maintain individualized Care Plans for patient management
• Conduct monthly care calls and monitor patient progress toward desired outcomes
• Communicate with the patient's clinical care team and address their needs and concerns

Required Qualifications

• Bilingual (English/Spanish) required
• Minimum of two years' experience providing patient-centered care
• At least one year of experience in skilled nursing
• Experience working with electronic medical records
• Ability to perform nursing assessments and develop care plans
