Job Summary

Key Responsibilities

• Develop and maintain individualized care plans for patients

Required Qualifications

• Bilingual in English and Spanish
• Minimum of two years' experience providing patient-centered care
• At least one year of experience in skilled nursing
• Experience with electronic medical records
