Job Summary

Key Responsibilities

Deliver telephonic care management for patients enrolled in Value-Based Care Programs
Develop patient-centered care plans and educate patients and caregivers on disease processes
Coordinate with providers and communicate health updates effectively to patients and caregivers

Required Qualifications

Active, unrestricted compact LPN license
2+ years of nursing experience in various care settings
Ability to obtain additional state licenses as needed
Experience with Electronic Health Records
Ability to work independently in a remote environment
