Job Summary

A company is looking for a Care Manager, LPN (Central Time Zone).

Key Responsibilities

Deliver telephonic care management for patients enrolled in Value-Based Care Programs
Review electronic health records to identify care gaps and develop patient-centered care plans
Communicate health updates and treatment plans between patients, caregivers, and providers

Required Qualifications

Active, unrestricted compact LPN license
2+ years of nursing experience in various care settings
Ability to obtain additional state licenses as needed
Experience with Electronic Health Records
Case Management experience preferred
