LPN Care Manager

Job Summary

A company is looking for a Care Manager, LPN (Eastern Time Zone).

Key Responsibilities

• Deliver telephonic care management for patients enrolled in Value-Based Care Programs
• Develop patient-centered care plans and educate patients and caregivers on their health conditions
• Coordinate with providers and support teams to ensure quality care and facilitate necessary medical orders

Required Qualifications

• Active, unrestricted compact LPN license
• 2+ years of nursing experience in various care settings
• Knowledge of pathophysiology and treatment protocols for common health diagnoses
• Proficient computer skills and ability to adapt to technology platforms
• Ability to work independently and as part of an interdisciplinary care team
