Job Summary

To support a growing value-based care program, the full-time LPN Care Manager will deliver telephonic care management for patients with complex health needs, collaborating with providers and interdisciplinary teams in a remote environment.

Key responsibilities
  • Review electronic health records to identify care gaps and develop patient-centered care plans
  • Coordinate with providers and support teams to schedule patient visits and facilitate orders and referrals
  • Communicate health updates and treatment plans effectively to patients and their caregivers
Required qualifications
  • Active, unrestricted compact LPN license
  • 2+ years of nursing experience in inpatient, outpatient, or skilled nursing settings
  • Knowledge of pathophysiology and treatment protocols for common health diagnoses
  • Proficient computer skills and ability to adapt to various technology platforms
  • Demonstrated experience using clinical data to guide decision making
