LPN Case Manager

Job Summary

A company is looking for a Case Manager - LPN/LVN.

Key Responsibilities

• Manage cases on a short- and long-term basis over the phone
• Administer assessments to determine patients' healthcare needs and collaborate with RN Case Manager on treatment plans
• Educate patients on treatment options and coordinate referrals while monitoring the effectiveness of treatment plans

Required Qualifications

• Graduation from an accredited LVN/LPN program and a current LVN/LPN license
• Prior experience in case management or coordinating wellness programs
• Experience in medical, surgical, and/or acute care settings
• Strong understanding of case management and health program coordination
• Computer proficiency and working knowledge of Microsoft Office Suite
