LPN Case Manager Assistant

Job Summary

A company is looking for a Case Manager Assistant / LPN.

Key Responsibilities

• Assist the RN Case Manager with case management functions and maintain communication with healthcare team members
• Review medical data and contact providers and payers to update case management plans and verify medical necessity
• Document case actions in the computer system and prepare timely reports for the payer regarding case management activities

Required Qualifications

• Current active and unrestricted Licensed Practical Nurse or Medical Assistant/Nursing Assistant Certification
• Preferred education in Licensed Practical Nursing
• Basic computer skills
• 5 years of varied clinical experience preferred, or 3 years in a specialty field
