LPN Case Manager Assistant

Job Summary

To support a remote healthcare team, the full-time LPN Case Manager Assistant will assist the RN Case Manager in case management functions, including service authorization, treatment plan reviews, and ongoing communication with healthcare team members.

Key responsibilities

• Assist in case management functions under the supervision of the RN Case Manager, excluding initial assessments and care plan formulation
• Review medical data and communicate with providers and payers to update case management plans and verify medical necessity
• Document case actions and maintain billing in the computer system, ensuring timely reports and ongoing patient care coordination

Required qualifications

• Current and unrestricted Licensed Practical Nurse (LPN) license or Medical Assistant/Nursing Assistant certification
• Preferred completion of Licensed Practical Nursing education
• Five years of varied clinical experience or three years in a specialty field
• Basic computer skills for documentation and case management
• Familiarity with community resources and funding sources for patient assistance
