Key Responsibilities

• Manage and support patients' healthcare needs through virtual phone conversations
• Develop personalized care plans and document visits using technology platforms and EHRs
• Connect patients with resources and prepare them for appointments and procedures

Required Qualifications

• Must have a clear and active Compact Nursing License
• Must reside and work in the continental US
• Minimum of two years of clinical experience required
• Experience in care coordination or health coaching is a plus
• Hands-on experience with Electronic Medical Records and Microsoft Office products
