LPN Escalation Nurse

Job Summary

Serving as an escalation point for care management and remote patient monitoring teams, the full-time LPN Escalation Nurse will resolve patient concerns within LPN scope of practice and escalate complex issues to the RN Escalation team while working remotely.

Key responsibilities

• Serve as an escalation point for patient concerns that exceed initial caller's scope of practice
• Rapidly review patient history and RPM readings to assess urgency and determine appropriate next steps
• Document all escalation activities, assessments, and outcomes thoroughly and compliantly

Required qualifications

• Current active, compact LPN/LVN license
• LPN/LVN degree from an accredited nursing program
• Strong triage and critical-thinking skills
• Ability to work independently and exercise sound clinical judgment
• Excellence in following established clinical escalation protocols
