Job Summary

To support Medicare patients throughout their healthcare journey, the remote LPN Healthcare Advocate will guide individuals through complex health concerns, develop comprehensive care plans, and build trusting relationships while working on a flexible contract basis.

Key responsibilities

• Learn and utilize Solace's proprietary system and tools to enhance patient interactions
• Identify patients' needs and create tailored care plans addressing their health and social determinants of health
• Build strong relationships with patients through empathetic listening and support

Required qualifications

• LPN/LVN license in good standing
• Experience working with diverse patient populations
• Strong clinical and organizational skills
• Ability to quickly adapt to new technologies and systems
• Demonstrated empathy and a commitment to patient advocacy
