LPN Intake Coordinator
Job Summary
Managing the intake process for new patients, the full-time LPN Intake Coordinator will facilitate referral management, insurance verification, and initial assessment scheduling in a remote setting.
Key responsibilities
- Manage the intake process, including receiving referrals, verifying insurance, and scheduling assessments
- Serve as the primary contact for patients and healthcare professionals regarding Home Health and Hospice services
- Ensure accurate documentation of patient information and collaborate with the interdisciplinary team to develop care plans
Required qualifications
- Associate's Degree or Certification in Nursing
- Current Licensed Practical Nurse license in the state of service or multi-state license through the enhanced Nurse Licensure Compact (eNLC)
- Two years of Home Health and/or Hospice experience
- Two years of experience in utilization review/chart audit
- Bachelor's degree in nursing is preferred
