Job Summary
To provide urgent patient support, the full-time LPN Licensed Care Advocate will manage triaged cases, develop execution plans, and ensure seamless communication with patients and the operations team in a remote setting.
Key responsibilities
- Take assigned cases and build execution plans based on urgency and patient needs
- Document interactions and support activities accurately to facilitate seamless handoffs
- Maintain communication with patients to update them on progress and gather necessary information
Required qualifications
- An active LPN/LVN license or Community Health Worker (CHW) certification, or equivalent experience
- 2+ years of patient- or community-facing support experience
- Working knowledge of the U.S. healthcare system and insurance
- Ability to manage multiple urgent cases independently while maintaining strong documentation
- Reliable computer, internet, and a quiet space for patient calls
