Job Summary
A company is looking for a LPN/LVN Remote Patient Monitoring with NY licenses.
Key Responsibilities:
- Serve as the point of contact for patients enrolled in the RPM365 platform by their health care professional
- Engage regularly with patients to encourage consistency with the treatment plan and ensure they remain within the data range set by their provider
- Educate patients through shared information and inform healthcare providers of monthly progress
Required Qualifications:
- Possesses a Nursing (LPN, LVN, RN, and/or Clinical Certification
- Knowledgeable about chronic health conditions and related devices
- Proficient with technology and smart phone functionality
- Experienced with client interaction
- Highly organized, efficient, and detail-oriented
