Job Summary

A company is looking for a Nurse Coach - LPN/LVN.

Key Responsibilities

Manage prescriptions for patients seeking obesity reversal in collaboration with medical providers
Educate patients on treatment and assist them in their nutritional and behavioral journey
Support patients using motivational interviewing techniques and collaborate with clinical teams to develop new service lines

Required Qualifications

Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
Current, active, and unencumbered LPN or RN license in a Nursing Licensure Compact state
Minimum of two years of experience working with diabetes patients in a clinical setting
Knowledge of low carbohydrate nutrition and meal planning
Interest in working with a tech-enabled healthcare organization
