Required Qualifications

• Active LPN or RN license in good standing
• Previous experience in customer service or healthcare roles
• Proficiency with EHR systems
• Experience in Chronic Care Management or population health programs preferred
• Basic understanding of medical billing or Medicare CCM requirements preferred
