Job Summary
A company is looking for a Bilingual Personal Care Coordinator I.
Key Responsibilities
- Provide monthly care coordination through planning, facilitation, and advocacy for patients' health needs
- Identify patient-specific problems and design action-oriented interventions based on clinical assessments
- Maintain patient chart compliance through proper documentation and updates of medical history and care plans
Required Qualifications
- Fluent in Spanish and English
- Licensed and registered as a Licensed Practical Nurse in a COMPACT state
- LPN degree from an approved program required
- Must pass a background check
