LPN Remote Care Counselor

Job Summary

A company is looking for an LPN, Remote Care Counselor.

Key Responsibilities

• Facilitate patient enrollment in Remote Therapeutic Monitoring (RTM) or Chronic Care Management (CCM) programs
• Make daily calls and texts to document patient health status and engagement
• Assist in collecting and documenting patient health information while maintaining confidentiality

Required Qualifications, Training, and Education

• Certification as an LPN
• 3+ years of experience working as a Medical Assistant in office or remote settings
• Proficiency with technology platforms such as Zoom, Slack, and online enrollment systems
• Familiarity with HIPAA regulations and commitment to patient privacy
• Ability to work independently in a remote environment
