LPN/RN Telehealth Nurse

Job Summary

Supporting a telehealth pilot program, the fully remote Nurse, Pilot will engage patients, assist with care coordination, and contribute to the development of scalable care models while working as a contractor.

Key responsibilities

• Conduct telehealth outreach to patients, providing education, follow-up, and support aligned with care plans
• Monitor patient progress and assist with care coordination activities across the clinical team
• Collaborate with a multidisciplinary care team to support patient goals and outcomes

Required qualifications

• Active LPN or RN license with compact privileges
• Clinical experience as an LPN or RN
• Proficiency with electronic health records (EHR) and healthcare technology platforms
• Strong communication and patient engagement skills
• Ability to work collaboratively in a multidisciplinary clinical environment
