LPN Telehealth Nurse

Job Summary

Providing remote patient monitoring and support, the full-time Telehealth Nurse (LPN/LVN) will conduct regular phone visits to educate members on managing chronic conditions such as hypertension and diabetes while ensuring accurate documentation and fostering strong rapport with patients.

Key responsibilities

• Perform regular telehealth calls to a dedicated panel of members
• Provide medical, diet, and wellness education while ensuring accurate and timely documentation
• Drive clinical outcomes through member engagement and coordination, escalating cases to the Provider team as necessary

Required qualifications

• Current, active, unrestricted LPN/RN multi-state compact license in a nursing compact state
• Minimum of 2 years hands-on nursing experience; telehealth and chronic care management experience preferred
• EHR experience, specifically with AthenaHealth, is a plus
• Fluency in English and Spanish is highly preferred
• Reliable high-speed internet and comfort with navigating multiple computer applications
