Job Summary

A company is looking for an LVN - Home Health Recertification Review Nurse.

Key Responsibilities

Conduct thorough chart reviews for home health recertification based on Medicare criteria
Verify documentation supporting ongoing homebound status and clinical eligibility for recertification
Prepare structured summaries and recommendations for RN program lead and provider review

Required Qualifications

Graduate of an accredited Licensed Vocational Nurse (LVN) program
Minimum of 5 years of experience in home health, care management, or related clinical field
Active, unencumbered LVN license in applicable state(s)
Familiarity with Medicare home health requirements and recertification processes
Proficiency with digital tools such as EMR systems and documentation platforms
