LVN or RN Post-Acute Care Manager

Job Summary

To facilitate seamless patient transitions through the Reserved Bed Program, the full-time Reserved Bed Post-Acute Care Manager will serve as the primary liaison between hospital partners and post-acute care centers, coordinating referrals, managing patient placements, and maintaining program metrics while working remotely.

Key responsibilities

• Educate hospital case managers on the Reserved Bed Program and manage the referral process to ensure timely patient placements
• Coordinate patient transfers to PACS facilities by arranging necessary documentation and services prior to admission
• Track and report key performance indicators related to referral activity, admissions, and program outcomes to ensure compliance and program growth

Required qualifications

• LVN or RN license required
• High school diploma or GED; Bachelor's degree in health care or related field preferred
• Two (2) years of Admissions experience preferred
• Proficiency in Microsoft Suite products and hospital EMAR systems
• Ability to maintain a professional and distraction-free remote work environment
